Experience: Prior direct care experience preferred, but not required.
Interpersonal Skills: Must be able to communicate effectively with all levels of personnel and individuals served.
Physical/Environmental: Must be able to lift, push and/or pull a minimum of 50 pounds, stand for periods of time, bend, push wheelchairs, and provide physical intervention on a regular basis. Noise level may vary, depending upon duty assigned.
Education: High School diploma or equivalent preferred.
Responsibilities: To provide quality living and community support and be responsive to individuals' needs in residential, work, or community settings. Teach, support and facilitate choices for individuals in their homes, work or community settings. Implement Person Centered Support Plans for assigned individuals. Assist with daily living skills including, but not limited to, meal preparation, general hygiene, toileting, general housekeeping, work and leisure activities. Administer medication and treatment(s). Complete documentation as required through the general course of duties. Report to the appropriate Program Manager, Case Manager or Wellness Liaison any occurrences that affect the immediate well-being of the individual. Attend all meetings as assigned and required. If approved as a driver of agency vehicles, employee may transport individuals. Other duties as assigned. Reference our website at dsgp.org for other expectations and qualifications. Other Skills: Basic mathematical, grammar and computer skills preferred. Valid drivers license preferred
